Student Athletes

As a Student Athlete at The College of Wooster, we require some pertinent information prior to their participation in our Inter-Collegiate Athletic Programs.  The following information and forms must be completed and returned to the Athletic Training Staff by July 15th, 2008.  No athlete will be able to participate in team practices or games until this information is received by the Athletic Training staff. 

1. Download Student Athlete Emergency Information sheet and fill in all blanks.  This form can be filled out on the computer by tabbing through the fields.  Print this form after completion.
2. Make a copy of both sides of your insurance card. Enlarge the card to approximately 150% of its original size. 
3. Print, read and sign the Athletic Insurance Coverage and Procedures form.  Both the athlete and the parent must sign this form. 
4. Mail the Athlete Emergency Information form, the Athletic Insurance Coverage and Procedures form and the copy of your insurance card to:
